Certified Occupational Therapy Assistant (COTA)

Full-time Flexible 8A-4:30P Days Qualifications:The COTA/L will hold a current license or is a licensure applicant by the OT section of the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. A graduate from an accredited university is considered a licensure applicant for 60 days from the time the State Board receives the fee for examination and the results are made public. Will be competent in the professional duties of treatments, note writing and patient/family education to all age groups. Must be able to lift and transfer patients up to 150#. Must be able to read, write and communicate effectively. In addition to communication with other therapy staff, internal communications include contact with patients, nursing staff, physician offices, and other hospital, clinic and homecare personnel. Any other external communication deemed necessary to expand or support OT services such as vendors and family members.

Position Summary:An Occupational Therapy Assistant provides for goal-directed purposeful activity to aid in the development of adaptive skills and performance capacities by individuals of all ages who have physical and related psychological impairments. The COTA/L will follow the treatment plan formed by the Occupational Therapist (OTR/L) at the initial evaluation. Documentation of patient progress notes, discharge notes, and daily notes will reflect complete and accurate data formulated in a traditional S.O.A.P. note (subjective, objective, assessment and plan). All written documents will be completed in a timely manner as described in the rehab policies and procedures manual. The COTA/L will be supervised by the OTR/L when providing patient care.
